Most of the wound looks like incisions to open it up and remove the bullet - without nicking the femoral artery. I suspect the bullet hole itself was just a small puncture wound.
In many cases, given that it causes more tissue damage to remove it than the bullet itself caused, surgeons will just leave a .22 LR bullet in place, like they would a small fragment or small piece of shrapnel if it's not going to become a problem.
